Are there any programs to create a scheme of transitions between web pages?

Question in two parts.

First part: how to properly called such diagrams (right part of the picture with a yellow background in the beginning of the article)?

The second part: what are some simple but quite functional program for the creation of such schemes?

The context of the question: participate in the development of applications with multiple screens. A simple circuit of switching between the screens sketched on paper, but I would like to create it completely in electronic form. To be able to sketch out the screenshots of the screens, connect them with arrows and that was as pretty and clear as the picture. :)

UPD: answers offer tools for prototyping. But I don't need to create prototypes. And interactivity is also not needed. I want to create a simple graph of transitions between pages so that all pages fit on one "canvas". In this form, as to me given the picture.
