How to put a condition for a checkbox?

I use:

if (!empty ($row[name3])) {

 echo "<div class="container"> 



 <figure><img src="templates/standart/img/testimonial2.jpg"></figure>
 <h5>" . $row['name3'] . "</h5>" . $row['otzov3'] . "<p></p>
</div>
";
 }

well, this is for input, the checkbox is empty the valuy therefore not suitable, I want when you click on the checkbox if it is selected, the output is from the database and if not then no output , how to do?
